Supportive In Home: MCSS renders these services in the individual’s home, family home, or community

  • Support in sensory stimulation and gross and fine motor skills
  • Support in the use of community resources, community safety, appropriate peer interactions and social skills
  • Support with adapting behavior to social and community settings
  • Support in behavior intervention and anger management
  • Support in skills which are aimed at preparation for enhancing life skills in a variety of settings
  • Support in various areas of skill building such as health, safety, and welfare, domestic skills, behavioral skills, self-help skills, communication, and community integration skills.
  • Support with use of community resources and opportunities to utilize skills in a variety of community venues.
  • Support with transportation of individuals between their home and community activity sites
  • Specialized supervision to ensure an individual’s health, safety, and welfare.
  • Completion of Assessments and Evaluations to determine the individual’s talents / contributions, interests, current resources, barriers, and needs.     
  • Completion of monthly reports, quarterly and annual person-centered reviews of all supports provided to determine an individual’s satisfaction and progress in meeting Person Centered Plan desired outcomes.  
  • Facilitate the involvement of the individual’s family and / or AR, when applicable for service planning as an equal voice with other professional staff input.                  
  • Development and implementation of PCP’s and program desired outcomes daily, weekly, or as needed per individual with a staff to individual ratio of 1:1 for both in-home residential and community settings.
  • Transportation to shopping centers, restaurants, and various social and community-based activities.  
  • Community Integration-Teaching an individual how to make a personal purchase of their choice and interest or access various services within the community i.e., using banking facilities, using a public restroom, ordering from a restaurant etc.
  • Health-Teaching an individual how to take their medication, or manage their diabetic condition through proper diet, nutrition, and exercise.   
  • Socialization-Teaching an individual how to develop a social life for meaningful activities such as, sporting events, restaurants, movies, etc.
       

Community Engagement Services include community education or training, retirement, and volunteer act

  • Community engagement service means a service that supports and fosters an individual's abilities to acquire, retain or improve skills necessary to build positive social behavior, interpersonal competence, greater independence, employability, and personal choices necessary to access typical activities and functions of community life such as those chosen by the general population.
  • The community engagement service may include community education or training and volunteer activities.
  • The community engagement service shall provide a wide variety of opportunities to facilitate and build relationships and natural supports in the community, while utilizing the community as a learning environment.
  • These activities are conducted at naturally occurring times and in a variety of natural settings in which the individual may actively interact with persons without disabilities, other than those who are being paid to support the individual.
  • The activities shall enhance the individual's involvement with the community and facilitate the development of relationships and natural supports.
  • The community engagement service shall be covered in the FIS, CL, and BI waivers.
  • The community engagement service shall be provided in the least restrictive and most integrated community settings possible according to the individual's plan for supports and individual choice.
